For your convenience, you can download, print and complete your confidential health information questionnaire to your first visit. Then bring the completed form with you when you come in for your appointment. If you choose to fill out your form at our office, please arrive 10-15 minutes before your appointment to allow sufficient time for completing your paperwork.
